Download and Pre-Complete NVL-019 Form
Fill in vehicle identification number, year, make, model, Nevada driver's license or ID number, and physical/mailing addresses. Review the reinstatement fee schedule showing fees of $250-$750 and fines of $250-$1,000 based on lapse length. Leave signature section blank.

Submit notarized NVL-019 to Nevada DMV Central Services and Records Division by mail (555 Wright Way, Carson City, NV 89711) or fax (775-684-4543). Include payment for reinstatement fees, fines, and SR-22 if applicable. Your registration will be reinstated upon processing.
