Submit NVL-019 at the time of reinstatement/renewal. If you go in person, you’ll sign before a DMV representative. If you reinstate by mail or fax, send an original, notarized NVL-019 with your Nevada Evidence of Insurance and payment. Online submission is generally not used for this affidavit.
