This is a jurat notarization, which requires you to take an oath. Our notary asks you to raise your right hand and swear (or affirm) that the statements in the VP-206 affidavit are true to the best of your knowledge. After taking the oath, you sign the document in the notary's physical presence. The notary witnesses your signature. This step is critical—the Nevada DMV will reject any VP-206 that was signed before the notary or without the oath.

The Registered Owner Must Sign: The VP-206 must be signed by the person listed as the registered owner on the vehicle title and registration. You cannot have someone else sign on your behalf—this is a sworn affidavit about your lost title, so you must personally appear before the notary. If there are multiple owners on the title, all owners must sign the VP-206.
Required Identification:
For Co-Owners: If the vehicle is owned by multiple people (shown as "and" or "or" on the title), all owners typically must sign the VP-206 affidavit. If the title says "John Smith OR Jane Smith," either owner can sign. If it says "John Smith AND Jane Smith," both must sign. We can notarize multiple owners in the same appointment.
For Business Vehicles: If the vehicle is registered to a business or corporation, an authorized company representative must sign the VP-206. Bring proof of your authority to act for the company (corporate resolution, authorization letter, or power of attorney from the business). You'll also need your personal photo ID.
Cannot Sign: The Nevada DMV will not accept VP-206 forms signed by unauthorized parties, individuals who are not the registered owner, or forms that were pre-signed before meeting with a notary. The oath and signature must occur in the notary's presence. If you're buying a vehicle from someone who lost the title, that seller must complete and notarize their own VP-206—you cannot sign it for them.
