The VP-241 Transfer on Death – Beneficiary’s Affidavit for Title lets the designated beneficiary claim a vehicle when all legal owners are deceased and the original certificate of title cannot be located. The form acts as a stand-in for the missing title and confirms the beneficiary listed on the DMV record.
VP-241 includes a hold-harmless statement and must be completed in ink with original signatures only. Photocopies or altered forms are not accepted. The affidavit must be notarized (Jurat) or witnessed by an authorized Nevada DMV representative.
