Nevada DMV Power of Attorney VP 136: when is it required and how must it be notarized?

VP 136 is Nevada DMV’s Power of Attorney used when the vehicle owner wants an agent to act on their behalf for title, registration, or related DMV tasks. It is commonly required when the owner is out of town, in a hospital or care facility, or delegating title work to a trusted family member.

  • Originals only: DMV requires wet ink signatures on the VP 136. Photocopies are not accepted.
  • Who signs: The principal (vehicle owner) signs and appears before the notary. The agent is named on the form but does not need notarization.
  • Certificate type: VP 136 uses an acknowledgment. We verify identity and willingness, then complete a Nevada-compliant certificate. If the POA is part of a sworn statement, a jurat may be used on the separate affidavit.
  • Scope and limits: The agent can sign DMV forms related to title and registration. This does not replace legal advice or court orders for complex estate or trust transfers.
