What supporting documents and steps are needed when submitting VP-202 for stolen plates?

For stolen plates, complete VP-202 and include the law-enforcement case number. Bring a valid photo ID and, if available, a copy of your police report. The DMV will cancel the registration/mark the plates as stolen and you’ll apply for a new plate number in person at a full-service DMV office.
