What supporting documentation is needed to submit the VP-159 form?

Submit a completed VP-159 together with a weight certificate or receipt from a state-certified scale. Sign your affidavit before a Notary Public or Authorized DMV Representative (original signatures only; no changes after signing). File in person at a DMV or, where applicable, the county assessor. If the unladen weight exceeds 26,000 lbs, file at a DMV with a Motor Carrier Division office. VP-159/weight certs are required at original registration or when the vehicle/combination changes—not for routine renewals.
